Everything in Normal Care, plus:

• An initial review by a psychologist experienced in dementia care, with an activity plan for your parent.
• Three 90-minute home visits each week by a dementia-trained care assistant. Basic activity materials are included.
• A monthly review of the activity plan by the psychologist.
• Guidance for family members and primary caregivers on communication, daily routines and responding to distress. This includes an initial session and one follow-up each month.

A live-in caretaker is optional and costs extra.

Everything in Normal Care, plus:

• Eight 60-minute sessions each month with a physiotherapist experienced in Parkinson's. These include the initial assessment and monthly progress review.
• An exercise plan to practise between sessions.
• Guidance for family members and primary caregivers on helping with the exercises safely.

Normal Care, with the following support agreed for your parent:

• A home assessment by a physiotherapist experienced in stroke recovery, following the hospital's discharge and rehabilitation advice.
• Home physiotherapy sessions at the agreed frequency.
• Speech therapy or occupational therapy when recommended. Occupational therapy helps with everyday activities such as dressing.
• Guidance for family members and primary caregivers on helping with exercises and everyday movements safely.
• Weekly coordination with the therapists and a progress review every four weeks.

Your quote will list the sessions included and the full price.

Normal Care, plus support for the agreed treatment period:

• Care Manager accompaniment for every scheduled cancer consultation, test and treatment visit in the agreed period.
• Help organising records and family questions before appointments.
• Help with registration, admissions and hospital paperwork.
• Next steps and family updates after visits, with help arranging follow-up appointments.
• Help contacting the treating hospital team when your parent or family reports a concern.

We confirm the visits and attendance hours with you before quoting. Cancer treatment and hospital charges are separate.

Normal Care, with the following support agreed for your parent:

• Accompaniment for every dialysis session in the agreed schedule, including registration, waiting and the return handover.
• Coordination with the dialysis centre when appointments or care instructions change.
• An initial consultation and monthly follow-up with a dietitian experienced in kidney disease, if your parent needs support beyond what the centre provides.
• Help contacting the dialysis team about concerns or unclear instructions between sessions.
